INSURANCE

Marble Counseling & Consulting, LLC may be able to bill some insurance plans for behavioral health services. However, participation in insurance networks is not guaranteed, and insurance coverage varies widely by plan and provider.

Marble Counseling & Consulting, LLC will bill insurance when applicable. 

Individuals must upload insurance information prior to services. 

Not all services may be covered by insurance.

It is your responsibility to verify with your insurance provider whether services are covered, whether prior authorization is required, and whether Marble Counseling & Consulting, LLC is considered in-network or out-of-network. 

While claims may be submitted on your behalf, this does not guarantee payment by your insurance company. You are responsible for any charges not covered by your plan, including deductibles, co-pays, co-insurance, or denied claims. 

If you have out-of-network benefits, you may request a superbill to submit for possible reimbursement. Payment is still due at the time of service. 

Some services may not be covered by insurance (e.g., late cancellations, administrative letters, legal proceedings). These fees are your responsibility.

It is your responsibility to notify this practice of any changes in insurance status or coverage. 

If you choose not to use insurance, a Good Faith Estimate of expected costs will be provided in accordance with the No Surprises Act.
